For clients who do not have private funding, and are funded through the health care system, follow the process below:

To receive service through the Brain Injury Services gateway you must:
- Be publicly funded through the health care system
- Have experienced a brain injury
- Be at least 16 years old
- Reside in the Hamilton, Burlington, Brant, Haldimand or Norfolk area
During the application process, the Admission Facilitator provides necessary assistance to applicants and their support network. While awaiting service, staff will provide case management services and assist with making appropriate referrals, as well as help provide short-term help. To determine if Brain Injury Services can provide the appropriate service, each application is reviewed on an individual basis by a committee.

Both of the forms linked below must be filled out to apply for service. These forms are in Adobe PDF file format. If you do not have Acrobat Reader installed on your computer, you will need to download it from Adobe. You can type directly on these forms or print them off and write on them. Alternatively, an application form can be mailed to you upon request.

Your doctor must fill out the Medical Status Form confirming that a brain injury is your primary diagnosis. Click the link below to access the form:
