Services & Programs
Outreach & Community Resource:
Through Outreach Services, adults living with the affects of an acquired brain injury in the South Central region of Ontario are offered opportunities for skill training and optimum community access. Whether individuals live on their own or in supported living environments, staff come to them. Outreach Services has offices in Hamilton, Simcoe and Hagersville.
Individualized treatment plans, with measurable outcomes, are developed for each client to assist with learning in the following areas: home management, banking, budgeting, physical activation, organization, time management, social skills, anger management, academic training, securing and maintaining employment or volunteer placements, community access, comprehension and assistance at medical appointments, and case co-ordination.

Outreach Services’ clients also have access to the other services offered by Brain Injury Services, Haldimand-Norfolk, Niagara, including the clinical services of a psychologist or behaviour therapist, therapeutic groups, group recreational activities, as well as a 24-hour help line.
The staffing team has a diverse range of expertise including employment counseling, community re-entry, psychology, recreation therapy and behaviour management. Each client works closely with an assigned case facilitator to pursue identified goals.

With the focus on empowerment, learning and increased opportunities, individuals involved in the outreach program achieve success. After achieving their personal goals, clients graduate from Outreach Services and transfer to Community Resource Services. This service meets the needs of individuals who have stable housing, financial management skills and a regular and satisfying routine but need assistance from time to time. Typically individuals receive one to four hours of service per month. clients can also transfer to Group Services to remain connected to recreational and volunteer activities. Others have chosen more independence and are discharged from the agency.

The opportunities for learning are extensive in Outreach and Community Resource Services. clients achieve their fullest potential, experience new opportunities, and are supported by families, the community and staff.
