Application Process For Services:
For clients who have private funding (typically through insurance companies or other sources), apply through the Step Up! ABI Recovery division. In addition to comprehensive adult programming, the Step Up! division also offers special services for adolescents:

For clients who do not have private funding, follow the process below:
During the admission process the Intake Facilitator provides necessary support to applicants and their support network. The Intake Facilitator provides case management services and assists with making appropriate referrals in the area. To determine if Brain Injury Services can provide the appropriate service, each application is reviewed on an individual basis.
To receive service you must:
- Have experienced an acquired brain injury
- Be at least 16 years old
- Reside in the Hamilton, Burlington, Brant, Haldimand or Norfolk area
Application Form For Services:
The application form is available in Adobe PDF file format. If you do not have Acrobat Reader installed on your computer, you will need to download it from Adobe. An application form can be mailed upon request.
